Prior to submitting your application, you will be provided a summary page with all of the details you have entered for final review. Please print a copy of this page for your records.

Please note that the fee for applying for reinstatement is $200. To submit your application, you will be asked to provide a credit card number that will be charged the non-refundable $200 application fee.
